Before you order a driver's PSP report, you need the driver's written authorization on the FMCSA form. With DOT Apply the driver e-signs it from a phone as part of the job application, and you get a signed PDF instead of paper and fax.

Part of DOT Apply, the free §391.21 driver application for small carriers.

FMCSA wording, word for word. The driver’s application on a phone, up to the signed PSP step. Each signature is stamped with IP address, time and a document hash.

What PSP is

What a PSP report shows, and the rules around it

PSP is FMCSA's Pre-Employment Screening Program. A PSP report shows a driver's crash history for the last five years and roadside inspection history for the last three. The program is voluntary.

In a PSP report

  • Crashes

    5 years
  • Roadside inspections

    3 years
  • 01

    Hiring only

PSP is for pre-employment screening, not for drivers you already employ.

  • 02

    Consent first

You need the driver's signed authorization before you order the report.

  • 03

    Keep it 3 years

Each signed authorization stays on file for three years, hired or not.

How it works

From your USDOT number to consent on file

  1. 01

    Set up your page

At peeledlab.com/onboard: pick your company from the FMCSA register and confirm your company e-mail and the phone number FMCSA has on file.

  2. 02

    Send one link

By text, on a job board, in a Facebook group, or embedded on your website.

  3. 03

    Driver applies from a phone

The CDL barcode fills in the license fields.

  4. 04

    Driver signs FCRA and PSP

The signed PDF lands in your cabinet.

  5. 05

    You order PSP

On your own PSP account, with the consent already on file.

Consent on file before you order PSP

The driver signs the FCRA disclosure, the FCRA authorization and the FMCSA PSP authorization, each one separately, stamped with IP address, time and a hash of the document. All three go into the same signed PDF as the §391.21 application.

Get the PSP form in your own driver application. Free.

We pull your company from the FMCSA register. You check it, add a logo, and get one link for drivers.

FAQ

PSP authorization, answered

Is the PSP authorization form in DOT Apply free?

Yes. The application page, the FCRA and PSP consent, the signed PDF and the cabinet are free.

Do I need the driver's consent before pulling PSP?

Yes. A PSP report requires the driver's written authorization on the FMCSA-mandated disclosure. DOT Apply collects it with its own e-signature, IP, timestamp and document hash, so it's in the file before you order anything.

Can I use PSP on drivers I already employ?

No. FMCSA allows PSP only for pre-employment screening, not for checking the records of current drivers.

How long do I keep the signed PSP form?

FMCSA requires PSP account holders to keep each signed authorization for at least three years, whether the driver was hired or not. With DOT Apply it is part of the signed PDF in your cabinet, and the PDF also arrives by e-mail.

Do you pull the PSP report for me?

No. You order it on your own PSP account. We don't pull MVR or Clearinghouse reports or run background checks either: we collect the consent and the application.

Can the driver sign on a phone?

Yes. The whole application, including the consent forms, is built for a phone. The driver scans the CDL barcode instead of typing the license data.

Is the form available in Spanish?

Yes. The application works in English, Spanish and Russian.

Can I put it on my company website?

Yes. Use the link or the embed code from your cabinet.

What else is in the signed PDF?

The full §391.21 application, the FCRA disclosure and authorization, the PSP authorization, and read-outs: what the CDL barcode says, gaps in the employment history, and a §391.23 previous-employer call list.
